Checklist for Embassy Appointment

Please look carefully through the enclosed information to make sure everything is correct. If it is correct, call the US Embassy in your home country to schedule an interview for your J-1 visa. Check-list of items to take to the Embassy

  1. DS-2019 CERTIFICATE OF ELIGIBILITY
  2. DS-156 NON-IMMIGRANT VISA APPLICATION FORM
  3. DS-157 SUPPLEMENTAL NON-IMMIGRANT VISA APPLICATION
  4. DS-158 CONTACT INFORMATION AND WORK HISTORY FOR NON-IMMIGRANT VISA APPLICANTS
  5. PHOTOGRAPH (white background, full head, no smiles, etc.)
  6. US EMBASSY BANK GIRO SLIP
  7. PASSPORT (Valid)
  8. PROOF OF RETURN to Home Country
    • Letter from employer that you are returning to work
    • Mortgage or Lease Agreement
    • Letter from University showing enrollment for Fall
    • Personal letter explaining legitimate reasons for return
